This happens in V25. Recently I am experiencing lead sheets where chord symbols pop up after copy/paste (of measures with no chords). Also, I may put in music using Speedy, no chord symbols yet, then I go to Chord tool and and as I use TAB to go from one measure to the next typing chords some measures already seem to have chord symbols attached that are only visible when highlighted. What's worse when I select these and delete them they are sometimes still there. 

 

I realize that may be due to me using a specific template file and I haven't yet detected a clear pattern when this happens. I think when I use Clear Measures with all Items Selected these things go away.

Just interested if anyone else has seen this.

For what it's worth, if I am understanding you correctly, this would be a new one for me. Your hunch that this could be happening due to an older template is a good thought. You may wish to check whether or not this happens in newly created documents.
