how to reproduce:

1) insert notes in separate measures;

2) merge measures (with Plug-in);

3) insert chord symbols;

4) copy and paste the final long measure;

If after there is a measure that have different key signature, the result is that the copy has not all chord symbols on the notes. The screenshot is more clear.
