What program and OS are you working with? Percussion has been revamped in the last few years. Have you looked over the tutorials and help files?
It looks to me as if the Lilypond stems are much thicker, and the noteheads are larger. That may be contributing to what you perceive.
What would be great would be if the sounds were only listed if they were actually there. And, if they were actually loud enough to hear.
There would not be a ledger line on the D, because it isn't necessary. Only when a note would go to where is needed - a C or a B, for example - would there be a ledger line. Just as there would not...
