Does it actually enter twice or is it a ghost image? Does the second image print with the page?
How exactly are you placing the graphic? Please list the steps you use. If you're following instructions from a Help page, please copy and paste those instructions.
I put a graphic into my score, it vanished. So, I put another one in. It vanished. When I went to make the score into a pdf, I have many graphics on the pdf, but do not know how to delete any of them! So frustrating!
