Probably because you have notes in both layer one and layer two. Deliberate or user error, I can’t say without seeing the piece, or at least, a screenshot.

The lyric issue is connected. You need to be in the correct layer that you want to attach the lyrics, to.

Red notes means that you're in layer 2. If you want to add lyrics to a melody, you need to be in the same layer as the music is in.

And if you don’t remember which verse and layer are connected, use the selection tool to double-click on a lyric in the score. It will put you in the correct situation.
