I suspect that it's because the lyrics don't match the notes they are attached to. If you can post a screen shot, we can probably tell you exactly where and why it's happening.

Nope. You don't need to leave any empty measures. You can move individual measures down with the down arrow, or use Utilities > Fit Measures to specify one measure per line.
