Nothing older than v. 26 is compatible with Mojave. You may need to find someone still running an older version, who can update the files for you. Sorry.

If you are starting from a template, you can set it up that way. I suspect you can do it in document styles, as well.

No idea. It might have been a bandwidth issue, in the early days, and would have taken too long to download.
